 One who, even during the time of departure, abandons his body while remembering me, he achieves my true nature, in this matter there is no doubt.

O Lord Kṛṣṇa, at this moment let the royal swan of my mind enter the tangled stems of the lotus of Your feet. How will it be possible for me to remember You at the time of death, when my throat will be choked up with mucus, bile, and air?

May the resplendent blue  effulgence of Gopāla reminding one of an incomparable blue gem stone ( blue sapphire), with blue peacock feather decorations on His dark black hair and the divine flute on His lips appear before me during the last moments of my life on this earth.

People generally depend on persons whom they think will help them when they become weak and old. Even though I am not qualified and eligible person, I have surrendered unto to you Oh my Lord. The other day you saved the Elephant Gajendra when it was caught by an alligator and cried out for your help. At the old age when I am fraught with disease I may not be able to call nay even think of you. Hence I would prefer to call you now itself when I am hale and healthy so that you can save me at my odd hours Oh my Lord Ranga Natha who is in reclining pose at divine Sri Ranga Shetra.

When all my senses have calmed down, When I have lost my intelligence,
When I am not capable of any movement, When my throat is full of phlegm,
When my body is shivering due to fear, And when my soul is preparing to depart,
Without any one to help me, Oh Lord of mercy, please come fast,
For I want to be in thine presence, Lord Guha. || 20 ||

If a person who while having mind and body in good shape with all the natural
elements in fine fettle, thinks of me as the birth less one, when such a person
reaches the time of death lying insentient like a log of wood or stone and
becomes unable to think, I will remember that person at that time and take him
to the highest state.
NOTE:
While lord Krishna mentioned that those who remembered him at the time of death only will be liberated, Sri Ramanuja got it clarified through Lord Varadaraja that this is not applicable to those who had surrendered to him earlier.

ThirukKachi Nambi obtained from Lord Varadaraja the famous ‘ Six Words’ and passed them on to Ramanuja. They provided the guidelines for Ramanuja to follow. They were:-

1.That Lord Narayana is the Paramaathma;
2.That the individual souls were different from Paramaathma;
3.That Prapathi is the means to attain salvation;
4.That the last remembrance of the Lord on the part of the departing soul was not necessary;
( अन्तिमस्मृति वर्जनं - Antima smruti varjanam )
5.That Moksha can be obtained only on laying off the mortal coils; and
6.That Ramanuja should take refuge at the feet of Periya Nambi.
